Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 24, 2023 from approximately 11:30 AM to 1:45 PM. There were 9 children in attendance and a total of two staff supervising. A total of five children and four staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-550-2-f
Based on review of four staff records and staff interview, the vendor did not obtain results of a sex offender registry check from any state in which one staff member resided in the preceding five years prior to beginning employment.
Evidence:
1. The record for Staff #3 (Date of Employment 10/16/2023) indicated Staff #3 lived in another state outside of Virginia in the past five years.
2. Staff #3 was present at the center caring for children.
3. The director confirmed the information during the inspection.
Plan of Correction: The out of state background checks were requested while the licensing inspector was onsite.

Based on review of four staff records and staff interview, the vendor did not ensure required records were kept for each staff person.
Evidence:
1. The record for Staff #4 (Date of Employment 08/01/2023) did not contain documentation of orientation.
2. A member of management stated the orientation had been completed but could not be located.
Plan of Correction: We will complete the orientation form again. Going forward, we will ensure all paperwork is file in staff records.
