Inspector Notes

An onsite code compliance inspection was conducted October 24, 2023. The inspection was initiated at approximately 11:30 AM and concluded at approximately 1:45 PM. There were 9 children present, ranging from 16 months to five years of age, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on review of four staff records and staff interview, the center did not obtain results of a sex offender registry check from any state in which one staff member resided in the preceding five years prior to beginning employment.
Evidence:
1. The record for Staff #3 (Date of Employment 10/16/2023) indicated Staff #3 lived in another state outside of Virginia in the past five years.
2. Staff #3 was present at the center caring for children.
3. The director confirmed the information during the inspection.
Plan of Correction: The out of state background checks were requested while the licensing inspector was onsite.
