Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician?s designee, or an official of the local health department. The record for Staff #1 contained a TB screening obtained 38 days prior to beginning employment.
Plan of Correction: Going forward, we will ensure the TB screening are obtained with the required timeframe.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment. A floor vent cover in one of the classrooms was rusting and warped, exposing it metal edges. Children were observed in the classroom.
Plan of Correction: We will replace the vent cover with a new one.

In areas used by children of preschool age or younger, electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Two classrooms were missing protective covers for electrical outlets. Children preschool age and younger were observed in the classrooms.
Plan of Correction: Outlet covers were removed because the outlets were used. Staff forgot to put the covers back in. We will remind staff to put the outlet covers back when electrical outlets are not in use.
