Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an on-site tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on October 5, 2021 and concluded on October 7, 2021. There were eight children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the vendor did not ensure that four out of five staff submitted documentation of a negative tuberculosis (TB) screening within the required time frame.

Evidence:
The record for Staff #2, hired on 09/29/2021, did not contain documentation of a negative TB screening. Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
