Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an on-site tour of the program.
A renewal inspection was initiated on October 5, 2021 and concluded on October 7, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were eight children present, ranging in ages from 18 months to four years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations

In classroom #2, a container of rubbing alcohol labeled ?Flammable," "Harmful if Swallowed or Inhaled? was in an unlocked cabinet. In classroom #3, hand sanitizer labeled "Flammable" and "Keep Away From Children" was on a shelf accessible to the children. In classroom #4, a container of Lemon Homescents disinfectant wipes labeled "Harmful" and "Hazards to Humans" and one can of spray disinfectant labeled "Danger," "Hazards to Humans" and "Keep Out of Reach of Children" was on the shelf of an unlocked cabinet. In the kitchen, one bottle of Multi-Purpose Cleaner, labeled "Hazard to Humans and Domestic Animals? and "Harmful"; seven containers of bleach labeled "Danger,? "Keep Out of Reach of Children" and "Harmful if Swallowed?; three containers of Betco Cleaner labeled "Danger" and "Hazards to Human and Domestic Animals" and one container of Comet labeled "Harmful if Swallowed" and "Keep Out of Reach of Children" were in unlocked base cabinets that were accessible to children.
Evidence:
On 10/05/2021, at approximately 10:10 AM to 10:19 AM, there was one staff member with 8 children in classroom #2. The youngest child (date of birth 04/05/20) was 18 months old.
Evidence:
1. The owner/director identified child #2, who was in care at the time of the inspection, as being 18 months in age when the license is issued for children 2 years through 12 years 11 months in age.
2. The owner/director verified that the child's birthday was 04/05/2020.
Evidence:
In classroom #3, children finished eating snack and began the next activity without washing their hands with soap and running water or disposable wipes.
Evidence:
1. The toilet chair located in the bathroom had a yellow substance in the chair. The children were in classroom #2 participating in circle time.
2. The center representative confirmed that the yellow substance located in the toilet chair was urine.
Evidence:
1. Child #2 did not have a record on file at the center.
2. Child #2 was observed in care at the center during the time of inspection.
Evidence:
1. Volunteer #1 (start date 9/29/21) was counted in staff-to-child ratio on 10/05/2021, and the center did not have documentation of the volunteer's completed fingerprint-based national criminal background check.
2. Owner/director confirmed lack of results in file, stating that she "hired a lady and she did not show up last week". The owner/director requested volunteer #1 to help at the center without obtaining the fingerprint-based national criminal background check.
