Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on June 26, 2024 and concluded on June 28, 2024. The inspector was on site on June 26, 2024, from approximately 4:00 PM to 5:46 PM, 18 children were present with three staff. The inspector was on site to complete renewal inspection on June 28, 2024, from approximately 11:15 AM to 1:20 PM, 20 children were present. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interviews, the vendor did not ensure that staff who work directly with children completed the department's health and safety update course annually.

Evidence:
The records for staff #1 (employed: 5/27/21) and staff #2 (employed: 9/29/21) did not contain documentation of the most recent annual health and safety update training. During interview, a member of management acknowledged staff #1 and staff #2 did not have documentation of required annual training.
