Inspector Notes

This inspection was conducted by licensing staff using an in person component, including email contacts, documents review, interviews and an in person tour of the program.
A monitoring inspection was initiated on 2/14/2022 and concluded on 2/14/2022. There were 43 children present, ranging in ages from 4 months to 5 years, with 11 staff supervising. The children were observed hand washing before morning snack, engaged in manipulatives, matching and working on a number line. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Based on a review of staff records, staff have not at least every two years from the date of the first initial screening or testing, obtain and submit the results of a follow-up tuberculosis screening.
Evidence: Staff #1 TB screening on file is dated 5/20/2019 and staff #2 TB screening is dated 9/17/2019, not within 2 years from the initial date.
Plan of Correction: Staff will get a current TB screening.
Standard 8VAC20-780-40-K
Based on a review of center procedures and Director interview, the center has not developed written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence: The director stated that she is not aware of written procedures for prevention of shaken baby syndrome or abusive head trauma.
Plan of Correction: The Director will speak with the owners to develop the written procedures.
Standard 8VAC20-780-510-F
Based on a review of medications, one child's medication authorization was not available to staff.
Evidence: Child #1 has a medication on site for a fish allergy. There was no medication authorization from the parent or physician to administer the medication in an emergency.
Plan of Correction: The parent and physician will complete a medication authorization form and keep on file at the school.
