Inspector Notes

A renewal inspection was conducted on 8/31/2023. There were 49 children directly supervised by 13 staff members. Children were observed engaged in individual activities, shapes, puzzles, matching colors, number lines, word box, matching animals and their babies and outdoor play time. The physical plant, playground, 4 child records, 4 staff records, medications, attendance, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on a review of staff records, one staff did not submit tuberculosis screening within the last 30 days of the date of employment.
Evidence: Staff #4, date of employment 8/7/2023 has a TB screening that is dated 10/8/2021, not within 30 days of first employment.
Plan of Correction: Director will ensure TB screening is obtained within 30 days of hire for all new staff hires.
Standard 8VAC20-780-240-B
Based on a review of staff records, 4 staff have not completed the required training in recognizing child abuse and neglect no later than 7 days of the date of assuming job responsibilities.
Evidence: The following staff do not have documentation that they have taken the training in recognizing child abuse and neglect no later than 7 days of assuming job responsibilities. These staff were observed caring for children this date.
Staff #1, date of hire 7/3/2023, staff #2, date of hire 6/1/2023, staff #3, date of hire 8/14/2023 and staff #4, date of hire 8/7/2023.
Plan of Correction: All staff with take the child abuse and neglect training and file the certificates of completion in their record.
Standard 8VAC20-780-510-I
Based on a review of children medications, 1 child's medication does not have the prescription label with the prescribers instructions pertaining to dosage, frequency and manner of administration.
Evidence: Child # 5 Auvi Q injection medication was not in the original container with the prescription label with the prescribers instructions pertaining to dosage, frequency and manner of administration.
Plan of Correction: The parents will provide the prescription label for the child's medication.
