Inspector Notes
A renewal inspection was conducted on 8/31/2023. There were 49 children directly supervised by 13 staff members. Children were observed engaged in individual activities, shapes, puzzles, matching colors, number lines, word box, matching animals and their babies and outdoor play time. The physical plant, playground, 4 child records, 4 staff records, medications, attendance, evacuation drills, and emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 3Evidence: Staff #4, date of employment 8/7/2023 has a TB screening that is dated 10/8/2021, not within 30 days of first employment.
Evidence: The following staff do not have documentation that they have taken the training in recognizing child abuse and neglect no later than 7 days of assuming job responsibilities. These staff were observed caring for children this date.

Evidence: Child # 5 Auvi Q injection medication was not in the original container with the prescription label with the prescribers instructions pertaining to dosage, frequency and manner of administration.
