Inspector Notes
An unannounced, on-site monitoring inspection was conducted 7/29/2024 from 11:45am - 3:10pm. The inspector reviewed compliance in the areas listed above. There were 72 children present and 12 staff. The inspector reviewed 7 children?s records and 6 staff records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, training logs, medication administration logs, authorization and permission forms), interviews, observations, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the
violation notice issued to the program.
Violations
7Evidence:
1) On the smaller playground, briars were growing through the chain link fence, broken lattice on the steps contained protruding industrial staples, and a child's bench with a missing board and rusted bolts was accessible to children.
2) On the large playground, the metal framed jeep has chipping paint and cracked/splintering wooden boards.
3) In the M2 classroom, there is crumbling dry wall and chipping paint on the wall near the bathroom from the door knob.
Evidence:
In the Little M1 classroom, an aerosol can of Lysol was found in the children's bathroom on a child accessible shelf.
Evidence:
The center maintained individual documentation of injuries but did not have a plan for injury prevention.

The Little M2 classroom did not have a bottle of soap and water to clean the diapering surface prior to sanitizing with bleach.
Evidence:
1) In the Infant 2 classroom, a child had an OTC skin product with manufacturer guidelines that stated "do not use on children under 2 years of age".
2) Infant 2 and Little M1 had expired Aquaphor and diaper cream.
Evidence:
In the Little M1 classroom, multiple diaper ointments and creams were stored on a low and accessible shelf in the children's bathroom.

Evidence:
1) In the Infant 2 classroom, bottles were labeled with each child's name but not dated. Several lunchboxes were not labeled with children's names or dated.
2) In the Little M1 classroom, 2 out of 3 lunchboxes were not labeled with a name or dated.
3) In the Little M1 classroom refrigerator, 4 yogurt and fruit pouches were not labeled or dated.
4) In the M1 classroom, 5 out of 7 lunchboxes were not labeled with a name. 7 were not dated.
5) In the M2 classroom, 1 lunchbox was labeled with a child's name but was not dated.
6) In the M3 classroom, lunchboxes were not labeled with names or dated.
