During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced renewal inspection was conducted on 8/12/2025. The insepctor reviewed 5 child records, 6 staff records, and 6 board member records on-site. The inspection included document review, interviews, observations, and a tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business 8/19/2025. A POC submitted after this date will not appear on the public website.
Violations
5The initial immunization record for Child #2 was dated almost 4 months after the first date of attendance.
An updated immunization record was not obtained for Child #4 until 46 days after it was due.
In the Infant classroom, a child was observed in a walker with wheels.
Child #2, enrolled for almost 2 years, and Child #3, enrolled for more than 2 years, did not have documentation of an annual update.
The record for Child #1, enrolled for 3 weeks, did not contain employment information for one parent.
