Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn disclosure statement prior to the employees first day of employment.

Staff #1, employed for 7 months, completed a sworn statement upon initial hire in 2023. Staff #1 separated from the center for more than one year, making the sworn statement no longer valid. The center did not obtain a new sworn statement upon rehire.
Plan of Correction: A new sworn statement will be completed today.

The center is required to obtain a completed national criminal background check prior to the employees first day of employment.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On the date of the inspection, Child #1, age 9 months, was observed sleeping in a crib with two mattresses. The staff present stated the child was able to sit and pull themselves to a standing position which poses the risk of the child falling over the crib rail.
Plan of Correction: The second mattress will be removed.

Children under 10 years of age always shall be within actual sight and sound supervision.

On the date of the inspection, the inspector observed the infant classroom during nap. The classroom lights were off and the blinds were down and closed, not allowing for proper sight supervision.
Plan of Correction: Classroom blinds are open to let more light to the room.

The required ratio for children age birth up to 16 months is 1 staff per 4 children.

On the date of the inspection, the inspector observed the infant classroom with 1 staff per 7 infants. The classroom was over ratio for approximately 15 minutes after the second teacher went on break.
Plan of Correction: We are keeping two teachers during the nap time.
