Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on 9/29/21 and concluded on 10/1/21. The provider was contacted by email on 9/30/21 to provide documentation to complete the inspection. There were five children present, ranging in ages from 15 months to three years, with the provider supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations

1
Standard 63.2-1721.1-B-2
Based on record review and interview, the provider failed to ensure a household member submitted fingerprint results within 30 days after turning 18.

Evidence:

1. Per documentation household member 1 turned 18 in June.
2. The provider verified household member 1 has not submitted fingerprint results.
Plan of Correction: An appointment for 10/2/21 has been set for household member 1 to get fingerprints completed. Once the results are received they will be forwarded to the licensing inspector.
