During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/26/2025. The on-site inspection began at 1:10 p.m. and ended at 2:00 p.m. The inspector reviewed compliance in the areas listed above. There were nine children present and two caregivers; the point total was 18. The inspector reviewed three children?s records and one caregiver record on-site.
This inspection included document review, tour of the facility and interviews

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/11/2025. A POC submitted after this date will not appear on the public website.

The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Child #1, first day of attendance 10/16/2024, did not have a physical in the record.
Plan of Correction: The parent was contacted during the inspection for the documentation.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Child #1, first day of attendance 10/16/2024, did not have documentation of immunizations or exemption in the record.
