Inspector Notes
An unannounced monitoring inspection was initiated and completed on June 10, 2025 in response to a self-report received by the licensing office on April 29, 2025 and on May 21, 2025 relating to administration, care of children and emergencies. The inspector reviewed one child?s record on-site June 10, 2025. This inspection included document review,tour of the facility, interviews, observations
