Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 12:10pm-2:25pm. There were 16 children in the direct care and supervision of 4 staff. A sample of 4
children?s records, and 4 staff records were reviewed. The children were observed napping, and during restroom procedures. One new staff member has been hired since the last inspection. Areas of non-compliance are identified in this report. Measurements were taken for the new space on floor 2. The exit interview was conducted with the director. Violations
5Evidence:
1. Staff A did not complete any hours of annual training in 2022.
2. Staff B completed 4 hours of annual training in 2022.
Evidence:
Cleaning supplies were found unlocked in both bathrooms that are occupied by children.
Evidence:
7 Children were out of actual sight and sound supervision for approximately 1 minute when Staff A took 1 child to the restroom.
Evidence:
The 911 and poison control numbers were not posted at the telephone located at the door of the "Toddlers" room.
Evidence:
The records of children A-C were missing a signature on the first page of the health documents.
