Inspector Notes
An unannounced monitoring inspection was conducted on 07/20/2022. There were 15 children, ages 4 years- 10 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 5 children?s records and 4 staff records were reviewed. The children were observed in the following activities: outside play, indoor free choice activities and during lunch. The inspector discussed the following with the staff: caring for preschool aged children, table sanitization and record keeping. The inspector arrived for the inspection at 09:15 am and departed at 11:35 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

1. The record for Staff 2 contained a documented hire date of 09/13/2021. There was not a central registry search available for review during the inspection.
2. The record for Staff 3 contained a documented hire date of 01/25/2022. There was not a central registry search available for review during the inspection.
3. The record for Staff 4 contained a documented hire date of 06/2021. There was not a central registry search available for review during the inspection.
Evidence:
1. There was an unlocked container of Member?s Mark Disinfectant Wipes on a shelf in the large front room that was accessible to children.
2. There was an unlocked container of disinfecting wipes and disinfecting spray on a shelf in the room located next to the restrooms that were accessible to children.
3. There was an unlocked container of disinfecting wipes on a shelf in the arcade game room that was accessible to children.
Evidence: There were 5 uncovered electrical outlets in large front room; there was a preschool age child observed in the large front room during the inspection.
Evidence:
1. The record for Child 2 did not contain the following information: name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the addresses for both designated people were incomplete); allergies and intolerance to medication or any other substances, and actions to take in an emergency situation; and documentation of child updates and confirmation of up-to-date information in the child?s record as required (the record did not contain documentation of a 2022 update).
