Inspector Notes

An unannounced monitoring inspection was conducted on 12/05/2023 from 2:25 pm to 3:30. There were 3 children in care, ages three to four years, in one group under the care of two staff. The Licensing Inspector reviewed three children?s records, two staff records, and other program records. The children were observed playing, washing hands, and having lunch.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center has not been maintained in clean, safe, and operable condition.

EVIDENCE:
In the corner of the game room near the entrance door, floor tiles are chipping and coming up.
Plan of Correction: The director will notify the landlord, who will repair the floor. While waiting for the repair, the spot will be blocked off with a chair or other furniture.
