Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A 60 day monitoring inspection was initiated on 6/9/21 and concluded on 6/15/21. The director was contacted by telephone to initiate the inspection. There were 24 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law in the area of administration and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence:
1. The record for child 1 (date of enrollment: 4/20/21) does not contain documentation of a physical examination.
2. The center director confirmed that the record for child 1 is lacking documentation of a physical examination.
Plan of Correction: The center responded with the following: The child has an appointment for a physical on 6/16/21. A copy of the physical will be obtained from the parent and will be placed in the child's record.
