During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 1/15/25 and was completed on 1/15/25. The on-site inspection began at 11:00am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 5 staff. The inspector reviewed 5 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition.

The landscape fabric underneath the mulch on the playground is protruding up from the mulch, creating a tripping hazard.
Plan of Correction: The center responded with the following: The center owner has been contacted and the landscaping fabric will be removed from the playground by the weekend.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

There is a spray bottle of bleach water hanging on a hook on the wall above the diaper changing station in the bathroom and was accessible to children in care.
Plan of Correction: The center responded with the following: The bottle of bleach water will be removed from the bathroom wall and placed in a locked area. Staff will be reminded to ensure all cleaning materials are locked up when not in use.

Children's hands shall be washed with soap and running water after toileting and staff shall wash their hands with soap and running water before and after a diaper change.

Staff 1 changed two children's diapers without washing her hands with soap and running water before and after each diaper change. In addition, staff 1 did not ensure that the children's hands were washed after their diapers were changed.
Plan of Correction: The center responded with the following: Staff will be retrained on required handwashing procedures.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

The trashcan in the infant room used to dispose of soiled diapers is not foot operated or used in a way that neither the staff's hand or the soiled diaper touches the exterior surface of the trash can during disposal.
Plan of Correction: The center responded with the following: A foot-operated hands free trash can will be purchased immediately and placed in the infant room.

Toilet chairs used for toilet training shall be emptied promptly and cleaned and sanitized after each use.

There was a toilet chair in the bathroom used by children that contained urine. The toilet chair was not emptied or cleaned and sanitized after use.
Plan of Correction: The center responded with the following: Staff will be retrained on policies regarding toilet training and toileting procedures.

Tables shall be sanitized before and after each use for feeding.

Staff 1 did not sanitize the table used for feeding children after lunch was concluded.
Plan of Correction: The center responded with the following: Staff will be retrained on food service procedures and the correct way to clean and sanitize tables after meals and snacks.

The center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for child 1, enrolled for over a year, does not contain the work address for the parent.
The record for child 2, enrolled for 4 months, does not contain the home address or the place of employment and work phone number for one parent or the work phone number for the other parent.
The record for child 3, enrolled for 4 months, does not contain the work phone number for the parent.
