During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and was completed on 6/25/25. The on-site inspection began at 1:30pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 6 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
The landscape fabric underneath the mulch on the playground is protruding up from the mulch, creating a tripping hazard.
Plan of Correction: The center responded with the following: The landscaping fabric has been removed from the playground.

A shady area shall be provided on playgrounds during the months of June, July and August. The playground did not have a shady area for the children.
Plan of Correction: The center responded with the following: A canopy has been purchased to provide shade on the playground during the summer months.

If diaper ointment or cream is used, these products do not need to be kept locked but shall be inaccessible to children. There was a tube of diaper ointment laying out on the open shelf of the diaper changing table in the infant room. The ointment was accessible to children in care.
Plan of Correction: The center responded with the following: Staff will be reminded to ensure diaper ointments are kept inaccessible to children after use.

The first aid kit shall include all the required items. The center's first aid kit did not include tweezers.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff 1, who has been working at the center for the past 3 months, did not have any completed reference verifications on file.
