During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and was completed on 10/1/25. The on-site inspection began at 10:35am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 5 children's records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Repeat Violation
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. The air conditioning vent on the lower wall between the infant room and the hallway is covered in rust. The corner of that same wall has been damaged and has exposed chipping drywall. The half door leading from the preschool room to the bathrooms is broken at the door handle and also has cracked and deteriorating wood. On the playground, there are branches with sharp thorns growing up and through the chainlink fence as well as exposed rusted nails on the wooden lattice fence.
Plan of Correction: The center responded with the following: Center director will contact the owner to have the above listed areas repaired.

Repeat Violation
Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. There was a can of lysol and a container of disinfecting wipes on an open counter in the toddler room. In the kitchen, there were containers of bleach and multipurpose cleaner sitting out on a shelf and on the floor. The door to the kitchen was open and accessible to children in care.
Plan of Correction: The center responded with the following: The cleaning materials on the counter in the toddler room were removed during the inspection and placed in a locked area of the center and the kitchen door was closed and locked. Staff will be reminded to keep all hazardous substances locked up when not in use.

Cleaning and sanitizing materials shall be stored in areas physically separate from food. In the toddler classroom, there were cleaning materials being stored on a counter that also contained food for children in care.
Plan of Correction: The center responded with the following: The cleaning materials were removed from the counter during the inspection. Staff will be reminded that all cleaning materials are to be stored separately from food items.

Repeat Violation
Children's hands shall be washed with soap and running water after toileting and staff shall wash their hands with soap and running water before and after a diaper change. Staff 1 changed two children's diapers without washing her hands with soap and running water before and after each diaper change. In addition, staff 1 did not ensure that the children's hands were washed after their diapers were changed.
Plan of Correction: The center responded with the following: Staff will be retrained on the center's policy on handwashing during diaper changes.

The diapering area shall have a sink with running warm water. The diapering area in the infant room does not have a sink readily accessible.
Plan of Correction: The center responded with the following: A portable sink will be purchased for the infant room.

Repeat Violation
Toilet chairs used for toilet training shall be emptied promptly and cleaned and sanitized after each use. There was a toilet chair in the bathroom used by children that contained urine. The toilet chair was not emptied or cleaned and sanitized after use.
Plan of Correction: The center responded with the following: Staff will be retrained on policies regarding toilet training and toileting procedures.

When centers choose to provide meals or snacks they should follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). On the day of the inspection, the children were not served milk during lunch, which is a required USDA component.
Plan of Correction: The center responded with the following: Staff will be retrained on following the USDA guidelines for meals and snacks.

No child shall be allowed to drink while walking around. A child in the infant classroom was observed drinking water from a plastic water bottle while walking around the room.
Plan of Correction: The center responded with the following: Staff will be reminded that children are not to eat or drink while walking around the classroom and retrained on the center's food service procedures.

When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. During the inspection, there were two infants sitting in floor seats eating their lunch. The protective belts were not fastened on either child.
Plan of Correction: The center responded with the following: Staff will be reminded to ensure the protective belt's on infant seats and high chairs are fastened when in use.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. During the inspection, there were 7 children present in the preschool classroom, 5 children present in the toddler classroom and 4 children present in the infant classroom. None of the children were marked present on the attendance records in any of the classrooms.
Plan of Correction: The center responded with the following: Staff will be retrained on the center's procedure for documenting children's attendance.
