Inspector Notes

An unannounced non-mandated inspection was conducted today, 1/08/2024, from 11:30am to 1:30pm. The inspection was in response to a complaint received on 01/02/2024 regarding Administration, Staff Qualifications, the Physical Plant and Special Services. The Licensing Inspector was accompanied by a Licensing Administrator for the duration of the inspection. The physical space, all snacks and 8 staff records were reviewed or inspected. The complaint was deemed Valid regarding Administration and Staff Qualifications. The complaint was deemed Not Valid regarding the Physical Plant and Special Services.

Based on record review, documentation to demonstrate that the individual possessed the education, certification, and experience required by the job position, was not available for review. Evidence: Documentation, demonstrating experience required by the job position, was not available for 1 out of 8 staff records. Staff B was missing documentation demonstrating programmatic experience.
Plan of Correction: We will obtain required documentation within 48 hours. Documentation for all future hires will be verified prior to hire.
