Inspector Notes

An unannounced, on-site renewal inspection was initiated on 10/14/2025 and completed on 10/14/2025. The on-site inspection began at 8:30AM and ended at 10:05AM. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 staff at the beginning of the inspection. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.

The 2 year follow up from the previous TB test, for staff #1, is overdue by 6 weeks.
Plan of Correction: We will schedule the TB test for today or tomorrow.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

The lists were posted in each room but in plain sight for anyone to see.
Plan of Correction: We will cover the allergy lists today.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Documentation of viewing proof of the child's identity and age.

Copies of birth certificates were in 3 out of 3 files, but the date the original was seen and who viewed the original certificate were not documented.
Plan of Correction: We will email all families to bring back originals to match the copies in the files. This time we will document the date we saw the original and note the signature of who saw the original.
