Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced renewal inspection was conducted at 12:53pm. Center management was contacted by phone to initiate the inspection. There were 74 children and 12 direct-care staff present. Ratios were in compliance at the time of the inspection. Children were observed taking naps. The infants were napping and being held by staff. The facility was found to be clean, and classrooms were well supplied with toys and equipment for the children. Information gathered during the inspection determined there was an area of non-compliance with applicable standards or laws reviewed. Based on records reviewed, it was determined that 1 of 2 children's records did not include immunizations every 6 months until the age of 2 years old. Evidence: Child #8 - The immunizations were dated July 2020, when the child was 12 months old.
