Inspector Notes

Conducted an unannounced monitoring inspection at 11:10am. Observed 89 children + 19 direct-care staff. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include: music, circle-time, learning about weather, outdoor active play, handwashing, eating lunch, and taking naps. Infants were being held, fed, napping, and having play time on the floor. Staff were actively engaged with the children and activities. Each child packs their own lunch daily. The facility was found to be clean and well supplied toys and equipment for the children. An area of non-compliance with standards and laws reviewed was found. Based on records reviewed, the licensee has not ensured that 2 of 4 employee records reviewed included completed out-of-state background checks prior to the employee's first day of employment.

Evidence:

1. Staff #1 - Their first day of employment was 11/11/22. To-date, an out-of-state criminal record check and a child abuse and neglect search have not been completed.

2. Staff #3 - Their first day of employment was 11/4/22. To-date, 2 out-of-state criminal record checks have not been completed, and 1 child abuse and neglect search has not been completed.
Plan of Correction: Staff #1 background checks for out-of-state have been requested on 5/24/23.

Staff #3 out-of-state background checks have been requested on 5/25/23.
