Conducted an unannounced complaint inspection into allegations of an unexplained injury to a child. Interviews conducted and records reviewed. While there was insufficient evidence to determine how and where the injury occurred, one area of non-compliance with standards and laws reviewed was found. Based on interviews conducted, records reviewed, and evidence collected, the licensee failed to ensure that suspected abuse was reported in accordance with mandated reporter requirements Code of Virginia 63.2-1509.
Evidence:
1. On 7/17/23, the parent of Child #1 contacted the school to express concerns about an injury found on the child. A photo was taken by the parents on the same date. [Photo #1]
2. On 7/18/23, the parents and school management met to discuss the concerns.
3. On 7/27/23, the inspector contacted Fairfax County Child Protective Services (local agency) to determine if the licensee had reported the concerns and was told that no report had been made. The inspector proceeded to report the concerns to the local agency.
