Inspector Notes
An unannounced monitoring inspection was conducted with the Assistant Director from 11:45 am to 1:15 pm. There were thirty five children present with four staff. Two groups of children were in the process of eating lunch at the beginning of the inspection while the third group of children were napping. The ages of the children ranged from 2 to 4 years.
Lunches are brought from home. The children were engaged in talking to their peers and teachers as they ate.
Seven staff files were reviewed as well as five children's files. One child had medication onsite along with an allergy care plan. The areas of non compliance are outlined on the violation notice.
Violations
3Evidence:
There was no orientation training on file for staff # 1 ( hired 8/21/23) and staff #2 ( hired 7/11/23).
The orientation provided to staff # 3 ( hired 6/20/23), #4 ( hired 7/14/23), #5 ( hired 6/23/22) and #6 ( hired 6/23/22) were missing the following components:
* emergency preparedness and response planning such as violence at a child day facility ;
* Use of safe sleep practices;
* Prevention of abusive head trauma, including procedures to cope with distraught children;
*Prevention and response to emergencies due to food and other allergic reactions; and
* Prevention and control of disease.
Based on observation, it was determined that the playground and its equipment did not maintain adequate fall zones or have a sufficient amount of resilient surfacing.
Evidence:
1. There were 3 spring rockers on the playground situated next to one another. The middle rocker was within 2'3" of the left rocker and was within 2"6" of the right rocker. All three rockers require at least 6 feet of fall zone.
2. The pole to the awning was within 2'6" of a see saw. The see saw requires at least 6 feet of fall zone.
3. There was one inch of resilient surfacing ( mulch) one foot in front of the end of the red slide. This slide requires at least 6 inches of resilient surfacing ( mulch).
Based on interview and a review of recordkeeping, it was determined that there was no documentation to show that the center implemented a monthly practice evacuation drill for July 2023.
