Inspector Notes

An unannounced renewal inspection was conducted with the director from 1:45 pm to 3:40 pm. There were thirty-seven children present with 6 staff providing supervision. The ages of the children ranged from 2 to 5 years. The children were observed napping and having snack.
Three medications were on site for 2 children, two allergy care plans were reviewed as well as 3 injury/accident reports.
Eight staff/agent files were reviewed as well as six children's files.
The most recent fire inspection was dated on 6/7/23 and the health inspection was dated on 2/21/23. The areas of non compliance are outlined on the violation notice.

Standard 22.1-289.036-A
Based on a review of 8 files and interviews, it was determined that 4 agents did not have repeat background checks every five years.
Evidence:
Person A's fingerprint background check was dated 7/12/18, Person B's fingerprint background check was dated 8/6/18, Person C's fingerprint background check was dated 7/19/18 and Person D's fingerprint background check was dated 7/13/18.
Plan of Correction: I will get the new fingerprint background checks. All Board Members ( Person A-D) background checks were submitted within 2 business days.
Standard 8VAC20-780-280-B
Based on observation, it was determined that hazardous substances were not kept locked.
Evidence:
There were 8 containers of Disinfectant Wipes and all purpose cleaner on a shelf in the office in the two year old room, the door to the office was unlocked.
There was an essential oil on a shelf near the entrance of the 2 year old room. The essential oil had warnings listed on the label.
Plan of Correction: We will lock all hazardous items.

Repeat Violation:
Based on observation and measurements, it was determined that the poles to the awning was within the fall zone of play equipment.
Evidence:
The 6 poles to the Awning was between 2 feet 11 inches and 4 feet 8 inches of the 3 Spring Rockers. The Spring Rockers require a 6 foot fall zone.
Plan of Correction: We will place the rockers off limits until we can remove the awning.

Based on observation, it was determined that a child was observed walking around while eating.
Evidence:
The Licensing Inspector observed 13- 2 year old children leaving their classroom, one of the children were eating their snack while walking.
Plan of Correction: We will make sure children do not walk or stand while eating or drinking.
