Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/9/24. The on-site inspection began at 10:20 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and 7 staff. The inspector reviewed 6 children and 6 staff records on-site. This inspection included documentation review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 22.1-289.035-B-4
Out of State Criminal History Record requests must be requested prior to employment.
There was no documentation to show that the Criminal History Record request was requested prior to Staff A's employment date, which was over a month ago.
Plan of Correction: We will make sure we document the request in the file.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis test/screening prior to employment. The test/screening shall have been completed within the last 30 calendar days of the date of employment.
1. There was no documentation of a tuberculosis test/screening on file for staff A who has been at the center for over 30 days
2. The tuberculosis test for staff B was dated over two years ago.
3. The tuberculosis test/screening for staff C was dated 16 months ago.
Plan of Correction: We will have staff obtain the tuberculosis screenings/tests.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff working alone with children and no later than seven days of the date of assuming job responsibilities.

There was no documentation that staff B and C had completed orientation within 7 days of assuming job responsibilities. Staff B has been employed for over 1 month and staff C has been employed for two weeks.
Plan of Correction: I will remember to document orientation training.

Repeat Violation:
Hazardous substances such as cleaning materials shall be kept in a locked place to prevent access by children.
The door to the office in the 2 year old classroom was unlocked and there were sanitizing wipes sitting on the desk and in reach of the children.
Plan of Correction: We will lock the hazardous substances today and remind staff to keep those items locked.
Standard 8VAC20-780-330-B
Repeat Violation:
Resilient surfacing shall be underneath and in the fall zones of the outside play equipment with moving parts or climbing apparatus.
There was 3 inches of resilient surfacing ( mulch )at the end of the red slide on the playground. This slide requires at least 6 inches of mulch in the fall zones.
Plan of Correction: We will rake the mulch in to the fall zones.
Standard 8VAC20-780-500-B
The diapering area shall be cleaned with soap and water and sanitized after each use.
After diapering a two-year-old child, the diapering area was no cleaned nor sanitized. There was no cleaning agents or sanitizing agent in the diapering area.
Plan of Correction: We will retrain staff in the proper procedure of diapering and of cleaning the diapering area.
Standard 8VAC20-780-510-E
The center's procedures for administering medication shall include duration of the parents' authorization for medication, provided that it shall expire or be renewed after 10 work days. Long term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent.
Medication authorizations for two over the counter medications for child A had not been renewed by the parent for over 5 months nor was their written authorization from the child's physician.
Plan of Correction: We will get the physician authorization.
