Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/9/24. The on-site inspection began at 10:20 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and 7 staff. The inspector reviewed 6 children and 6 staff records on-site. This inspection included documentation review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Violations
7There was no documentation to show that the Criminal History Record request was requested prior to Staff A's employment date, which was over a month ago.
1. There was no documentation of a tuberculosis test/screening on file for staff A who has been at the center for over 30 days
2. The tuberculosis test for staff B was dated over two years ago.
3. The tuberculosis test/screening for staff C was dated 16 months ago.
There was no documentation that staff B and C had completed orientation within 7 days of assuming job responsibilities. Staff B has been employed for over 1 month and staff C has been employed for two weeks.
Hazardous substances such as cleaning materials shall be kept in a locked place to prevent access by children.
The door to the office in the 2 year old classroom was unlocked and there were sanitizing wipes sitting on the desk and in reach of the children.
Resilient surfacing shall be underneath and in the fall zones of the outside play equipment with moving parts or climbing apparatus.
There was 3 inches of resilient surfacing ( mulch )at the end of the red slide on the playground. This slide requires at least 6 inches of mulch in the fall zones.
After diapering a two-year-old child, the diapering area was no cleaned nor sanitized. There was no cleaning agents or sanitizing agent in the diapering area.
Medication authorizations for two over the counter medications for child A had not been renewed by the parent for over 5 months nor was their written authorization from the child's physician.
