The vendor shall maintain, in a way that is accessible to all caregivers, a current
written list of all children's allergies, sensitivities, and dietary restrictions. This list shall
be dated and kept confidential.

The provider did not have a allergies, sensitivities, and dietary restrictions list. Child #4, who is enrolled has allergy listed on registration and in doctors documentation.

The vendor shall maintain a written hard copy record of daily attendance that
documents the arrival and departure of each child in care as it occurs. At 12:20 p.m. on 11/12/2024, there were 6 children present . The attendance was not taken for 11/12/2024.

Standard 8VAC20-790-250-B
Staff should complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Education within the first 90 days of employment or service all caregivers shall complete.

The family day home has been approved subsidy provider for 120 days staff #1 and staff #2 did not complete Virginia Preservice Training with in the 90 days of approval of service.
