Inspector Notes
A monitoring inspection was conducted jointly with local agency in response to a self-reported incident that occurred at the center on September 26, 2022. Inspection were conducted on October 12, 2022 and October 25, 2022. Interviews were conducted with staff, video footage was reviewed, documents were reviewed and observations of the program were made. Any violations found during the self-report inspection, are noted in the inspection.

Based on video footage, the licensee did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence: On September 26, 2022 the classroom was divide by shelving. The video camera on young 3?s classroom, showed children playing with no sight supervision. The video camera on the older 3?s classroom, Staff #3, Staff #4 and Staff #1 are sitting with children in the older 3's classroom. Staff #2 reviewed video footage and confirmed that there was no staff within sight of the children.

Plan of Correction: The classroom is no longer divided. It is one classroom with three teachers . The teachers are utilizing each other for the whole class. We had a staff meeting about appropriate handling children, procedures/steps put in place when teacher is leaving classroom,and be able to make sure everyone is visible to the teachers.
