Inspector Notes

An unannounced monitoring was conducted on June 06, 2023 from aproximately 12:05 pm-`1:24 pm. Children were observed napping and waking up from nap. Four staff records and four children's records reviewed . Records were reviewed remotley. Licensing Inspector reviewed indoor and outdoor areas, supervision, activities,
equipment,medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the licensee did not ensure the most recent violation notice was posted in a conspicuous place on the licensed premises.

Evidence: The violation notice posted was from October of 2021. The most recent inspection was conducted October 25, 2022.

Based on record review, the licensee did not ensure central registry findings results were recieved within 30 days of employment.

Evidence: The center did not provide the licensing inspector with the central registry results for staff #2 and staff #3 both hired in 2021.

Based on observation, the licensee did not ensure the areas and equipment of the center inside were maintained in a clean, safe and operable condition.

Evidence:
In the bathrooms down the right hallway the frame has peeling paint.
In the three year classroom, a vent cover was missing and there was hole.

Based on observation, the licensee did not ensure hazordous substances are kept in a locked place using a safe locking method that prevents access by children.

Evidence:
There was disinfectant aersol spray in the infant room and two bottles of handsantitzer at the entrance. Both have two warning labels.

Based on observation, the licensee did not ensure the staff to child ratio for birth up to 16 months- ratio 1:4 ( staff: children).

Evidence : On June 6, 2023, at 12:20pm there was 1 staff with 6 children. Confirmed with staff in the room.
