Inspector Notes
An unannounced monitoring was conducted on June 06, 2023 from aproximately 12:05 pm-`1:24 pm. Children were observed napping and waking up from nap. Four staff records and four children's records reviewed . Records were reviewed remotley. Licensing Inspector reviewed indoor and outdoor areas, supervision, activities,
equipment,medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
7Evidence: The violation notice posted was from October of 2021. The most recent inspection was conducted October 25, 2022.
Evidence: The center did not provide the licensing inspector with the central registry results for staff #2 and staff #3 both hired in 2021.
Evidence:
In the bathrooms down the right hallway the frame has peeling paint.
In the three year classroom, a vent cover was missing and there was hole.
Evidence:
There was disinfectant aersol spray in the infant room and two bottles of handsantitzer at the entrance. Both have two warning labels.
Evidence : On June 6, 2023, at 12:20pm there was 1 staff with 6 children. Confirmed with staff in the room.
Evidence:
In the infant room, their was a tear on the diapering surface.
Evidence:
The emergency drill docuemtation provided for 2022 did not have documentation of a lockdown drill that is doane annually.
