Inspector Notes
A monitoring inspection was initiated and concluded on 9/18/2023. There were 48 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed. The inspection started at 12:00 pm and concluded at 3:40 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations

Six staff records were reviewed.
The record for staff #1, whose date of hire is 3/33/2023, contained a sworn disclosure statement indicating that staff #1 resided in Massachusetts within the preceding 5 years.
The record for staff #1 did not contain the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from Massachusetts.
The program director confirmed that these background check results had not been obtained.
The record for staff #5, whose date of hire is 5/16/2022, contained a sworn disclosure statement indicating that staff #5 resided in Missouri within the preceding 5 years.
The record for staff #5 did not contain the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from Missouri.
The program director confirmed that these background check results had not been obtained
Evidence:
Six staff f records were reviewed.
The record for staff #6 contained the most recent Tuberculosis screening which was dated 5/26/2021, more than two years ago.
The program director confirmed that there was not a more recent screening result available for staff #6.
Based on observation, the licensee did not keep all hazardous substances such as cleaning materials, insecticides, and pesticides in a locked place using a safe locking method that prevents access by children.
Evidence:
The licensing inspector observed unlocked chemicals to include disinfecting spray, hand sanitizer, surface cleaner, dish detergent and glass cleaner in unlocked locations accessible to children in care, in classrooms throughout the center.
The labels on these products each contained a warning to "keep out of reach of children" as well as additional warnings.
These chemicals were observed under the diapering table in the Infant Classroom, under the diapering table in the toddler classroom, under the diapering table in the Two's classroom and in an unlocked cabinet in the Three's classroom.
The program director confirmed the presence of these chemicals in unlocked locations during the inspection.
