Inspector Notes
An unannounced monitoring inspection was conducted on July 14, 2023, in response to a self-reported incident that was received on July 13, 2023 in reference to supervision. Interviews were conducted jointly with local agency and documentation was reviewed. The preponderance of evidence gathered during the investigation supports violation of standards regarding the self-report.
Violations were cited.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 08/15//2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
5Based on record review, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record for staff # 3, date of employment 07/05/2023, contained documentation of a negative tuberculosis screening dated 07/17/2023.
Based on documentation review, and interviews, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
1. Interviews with staff #4, staff #5, staff #6, revealed that the two gates on the preschool playground have not been operating properly since April of 2023.
2. Interviews and documentation review revealed that work orders have been put in to fix the gates.
Based on interviews and documentation review, the center failed to ensure the care, protection, and guidance of three children.
Evidence:
1. Staff#1 and staff#2 took 8 toddlers outside to the toddler playground at approximately 10:45am.
2. Staff#4 found 3 of the toddlers (child#1, child#2, child#3) unsupervised on the preschool playground at approximately 11:10am.
3. Staff #1 and staff #2 were unaware that 3 of 8 toddlers were missing from the group until Staff#4 returned the children to the toddler playground.
All staff members retrained on child guidance. All staff retrained on the importance of sight and sound supervision. VDH Mandated reporter training completed on 8/16/23."
Based on interviews and documentation review, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
1. Review of name to face tracking sheet revealed that classroom #1, with a group of 8 toddlers and 2 staff, went outside to the toddler playground at approximately 10:45am.
2. Child#1 (23 months), child#2 (21 months), and child#3 (24 months) exited the toddler playground through an unlocked gate, walked down a passageway on the side of the building, and entered the preschool playground through a second unlocked gate.
3. Staff#4 found child#1, child#2, and child#3 unsupervised, out of sight and sound supervision, on the preschool playground at approximately 11:10am.
4. Staff #4 brought the three toddler children back to the toddler class on the toddler playground.
All staff members retrained on child guidance. All staff retrained on the importance of sight and sound supervision. VDH Mandated reporter training completed on 8/16/23."
Based on documentation review, and interview, the center failed to ensure that the center's own policies and procedures are followed.
Evidence: Interviews with staff #1, staff #2, staff #3, staff #4, and staff #5 revealed that on 07/14/2023, staff #1 and staff #2 brought 8 toddler age children out to the playground at approximately 10:45 am. Child #1 (23 months), child #2, (21 months), and child #3 (24 months), walked through one unlocked gate located on the toddler playground, down a passageway on the side of the building, through another unlocked gate located on the preschool playground and were found alone on the preschool playground by staff #4 at approximately 11:10 am. Review of the centers "Playground Safety" policy states "supervision with appropriate staff/child ratios is maintained at all times while on the playground" and "name to face attendance using the CSR is conducted on a regular basis so that every child is accounted for at all times."
All staff members retrained on child guidance. All staff retrained on the importance of sight and sound supervision. VDH Mandated reporter training completed on 8/16/23."
