Inspector Notes
An unannounced monitoring inspection was conducted on April 24, 2024 from approximately 10:30 am to 1:30 pm. There were 100 children in attendance and a total of 18 staff present. 4 staff records and 10 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed playing with bubbles, listening to a story, playing outside, and getting ready for lunch, while the licensing inspector was on-site.
The most recent evacuation drill was conducted on 03/13/24.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 04/29/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Evidence: The record for staff #4, date of employment 6/29/22, did not contain documentation of completion of the VDOE sponsored orientation course. Staff #4 was observed in ratio in a classroom with children. Interview with administration confirmed that staff #4 did not complete the VDOE sponsored orientation course.
