Inspector Notes
An unannounced monitoring inspection was conducted on June 13, 2024 from approximately 9:45 am to 1:00 pm. There were 89 children in attendance and a total of 17 staff present. 9 staff records and 9 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed dancing, learning about insects, practicing for graduation, playing outside, and playing in centers, while two licensing inspectors were on-site.
The first aid kit was inspected and found complete. Evacuation drills were conducted monthly with the most recent conducted on 05/23/24.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 06/20/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).
Violations
4Evidence: The record for child #8 (under the age of 2), date of enrollment 08/23/23, contained one set of immunizations. The record did not contain updated immunizations. Administration verified that there were no updated immunizations obtained for Child #8.
Evidence: There was a bottle of bleach in a low unlocked cabinet in classroom # 5 with children ages two to three years old.
Evidence:
1. Staff #1 was observed assisting a child with toileting in classroom #7. The child was handed a piece of candy after toileting. Staff #1 and the child did not wash their hands prior to being handed a piece of candy.
2. Staff #10 was assisting with toileting in classroom #6. Staff #10 did not wash their hands in between assisting the children with toileting.
Evidence: The refrigerator in classroom #2 contained a bottle for child #9, that was not dated. Child #9 was in attendance. Staff confirmed that the bottle was not dated.
