During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated on 07/24/25 and completed on 07/28/25, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to allergy management. The on-site inspection began at 11:30 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 8 children?s records on-site and electronically on 07/28/25. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 08/04/25. A POC submitted after this date will not appear on the public website.
Violations
3Child #8, who has been attending for 10 months, did not have documentation of an immunization record on file.
As part of corrective action, the CD and AD have undergone retraining on student orientation procedures and child file documentation requirements.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #8 who has been enrolled for 10 months.
The CD and AD have been retrained on the correct procedures for conducting student orientations and maintaining complete and compliant child files. They will be responsible for verifying the physical examination documentation that is received and filed before a child begins care.
1. Child #8, who has been enrolled for 10 months, has a diagnosed food allergy and did not have a written care plan in their file.
The child?s parent has requested the required documentation from the physician. The Center Director (CD) and Assistant Director (AD) will be responsible for obtaining this documentation during the orientation process, prior to the child?s first day of attendance.
