Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A focused monitoring inspection was initiated on 03/24/2022 and concluded on 03/25/2022 in response to an Intensive Plan of Correction (IPOC) presented to the facility on January 25, 2022 and completed/accepted on February 23, 2022. The director was contacted by telephone and a virtual inspection was conducted. The facility submitted documentation to the inspector on 03/25/2022. The licensing standards addressed in the plan included: 8VAC20-770-60-C-2; 22.1-(14.1)-289.035-B-2; and 22.1-(14.1)-289-035-B-4. A total of three staff background checks and orientation training were reviewed.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
