Inspector Notes
The licensing inspector initiated an unannounced monitoring inspection in response to a self-reported incident regarding a child?s injury that was received by the Department on June 30, 2022. The investigation was initiated on July 6, 2022 and completed on September 26, 2022. On July 14, 2022, the licensing inspector conducted an unannounced joint inspection with a local agency from approximately 11:00am to 1:15pm. Interviews were completed, observations were made at the center, and documentation was reviewed. Additional interviews were conducted on July 6, 2022 and July 27, 2022. On September 8, 2022, the inspector was on site from approximately 9:20am to 11:40am conducting additional interviews. No violations were found related to the self-report. Unrelated violations of standards were found during the course of the investigation and are identified on the violation notice.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Violations
1Evidence: 1) Staff interviewed reported that on 06/29/22, Staff #4 was observed ?yelling? and ?raising her voice? in front of the children at other staff members when inquiring about a child?s injury (Child #2).
2) Staff interviewed reported that Staff #2 was observed saying ?profanities? and ?yelling? in front of the children after Staff #4 confronted the staff. 3) While in front of the children, Staff #2 was observed saying to Staff #4: ?You?re not going to talk to me like that. I can go the [expletive] home? and proceeded to leave the facility.
