Inspector Notes

A complaint investigation was initiated on 08/31/2022 and concluded on 09/27/2022 in response to a complaint that was received by the department on 08/22/2022 regarding supervision, injury reporting, and administration. The inspector conducted an unannounced inspection on-site on 09/08/2022 from 9:20am to approximately 11:40am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. Unrelated violations of standards were found during the course of the investigation and are identified on the violation notice.

Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on a review of one child?s record and interview, the center did not ensure one child had received the immunizations required by the State Board of Health before the child attended the center.

Evidence: 1) The record for Child #1, date of attendance 07/28/22, did not contain documentation of the child?s immunizations. 2) During interview, Staff #1 confirmed the immunizations for Child #1 were not obtained before the child attended the center.
Plan of Correction: Per the Center: "Center will ensure that all immunizations required by the State Board of Health are received before the child enters care. Immunization records will be reviewed by center director/assistant director."
