Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint received by the department on 12/22/2021 regarding allegations of forbidden action, injury reporting, diapering, and the supervision of children. The investigation was initiated on 12/30/2021 and completed on 09/09/2022. On 12/30/2021, the inspector was on site with local agencies from approximately 12:00pm to 1:40pm. Interviews were completed, observations were made at the center, and documentation was reviewed. Additional interviews were completed off-site. The preponderance of the evidence gathered during the investigation supports part of the allegations, therefore the complaint is determined to be valid. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Based on documentation reviewed, the center did not maintain a written record of children?s serious and minor injuries with the required information.

Evidence: A sample of 11 incident reports written for Child #1 were reviewed. The following was not documented:

1) One written incident report reviewed did not include the date the injury occurred; the date and time when parents were notified; any future action to prevent recurrence of the injury; staff and parent signatures or two staff signatures; and documentation on how parent was notified.
2) Two written incident reports reviewed did not include the date and time when parents were notified; any future action to prevent recurrence of the injury; staff and parent signatures or two staff signatures; and documentation on how parent was notified.
3) Eight written incident reports reviewed did not include any future action to prevent recurrence of the injury; the date and time when parents were notified; and documentation on how parent was notified.
Plan of Correction: Staff received training on writing incident reports and how to document injuries on the day they occur. Management will ensure two staff or a parent and staff sign off on incident reports when they occur.
