Inspector Notes
An unannounced monitoring inspection was conducted on 12/16/2022. The inspector was on site at the center from approximately 10:35am to 2:15pm. There were 43 children in attendance and a total of 11 staff present. Interviews were held with staff throughout the inspection, and the inspector interacted with children in the classroom when appropriate. Six children?s records and seven employee records were reviewed during this inspection. There were violations found during today?s inspection. See the violation notice for more details.

1. The record of Household Member #2 (employment date: 12/12/2022) did not have documentation of fingerprint based national criminal history search results.
Evidence:
1. The record of staff #1 (employment date: 12/12/2022) did not have documentation of a completed sworn statement or affirmation.
Evidence:
1. The record of child #1 (enrollment date: unknown) did not have documentation of immunizations.
Evidence:
1. The record of child #1 (enrollment date: unknown) did not have documentation of a physical examination.
Evidence:
1. The record of staff #1 (employment date: 12/12/22) did not have documentation of orientation training.
Evidence:
1. There were six bottles containing formula located in the infant classroom not dated.
Evidence:
1. Child #3 was observed drinking a bottle that contained a semisolid food mixed with formula.
2. There was a bottle located in the infant refrigerator that contained a semisolid food mixed with formula.
3. The center director stated that the center did not have written instructions from a physician or physician?s designee regarding child #3 drinking semisolid food in a bottle.
Evidence:
1. The record of child #1 (enrollment date: unknown) did not have documentation of the following: child information, parent information, two required emergency contacts, child?s physician name and phone number, authorized to pick up information, allergies, health issues, school previously attended, required parent agreements and proof of identification.
2. The record of child #5 (enrollment date: 6/21/2021) did not have documentation of child updates and confirmation of up-to-date information and proof if identification.
3. The record of child #6 (enrollment date: 1/27/2019) did not have documentation of child updates and confirmation of up-to-date information.
Evidence:
1. The record of child # 1 (enrollment date: unknown) did not have documentation of a written allergy care plan. The center?s allergy list and the center director indicated that the child had a diagnosed food allergy.
2. The record of child # 2 (enrollment date: 11/15/2021) did not have documentation of a written allergy care plan. The child?s record and center?s allergy list indicated that the child had a diagnosed food allergy.
