Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by OCCHS on May 4, 2023. The inspection was initiated on May 15, 2023 and completed on May 30, 2023. On May 15, 2023, the inspector was on site from approximately 4:15pm to 5:00pm. Interviews with staff were completed and observations were made at the center. An additional interview was held on May 30, 2023. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
