Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated on October 10, 2023. The inspector was onsite from 10:45 am to 3:00 pm. Additional document was provided for remote review.The center director was available throughout the onsite inspection to answer questions.

There were 64 children present, ranging in ages 5 months to 11 years, with 10 staff supervising. A total of seven child records and nine staff records were reviewed.

During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children.

A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on review of staff records, the vendor did not ensure that each staff
complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department.

Evidence:
1. The record for Staff #3 (date of employment: 6/6/2022) contained documentation of the
Virginia Annual Health and Safety Update Course dated 3/8/2022.
2. The record for Staff #4 (date of employment:7/6/2020) contained documentation of the Virginia Annual Health and Safety Update Course dated 4/21/21).
3. The record for Staff #7 (date of employment: 4/25/2018) did not contain documentation of the Virginia Annual Health and Safety Update Course being completed since the date of employment.
4. The record for Staff #8 (date of employment: 8/30/21) contained documentation of the Virginia Annual Health and Safety Update Course dated 7/23/2020.
