During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 05/06/2025 and completed on 05/15/2025 in response to a complaint received by the licensing office on 04/18/2025 relating to allegations of behavior guidance and forbidden action. The inspector reviewed two children?s records and four staff records on-site. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/09/2025. A POC submitted after this date will not appear on the public website.

Complaint:
The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by the standards.

The center?s discipline policy for child guidance, behavior and classroom management states, ?The staff models positive behavior and uses methods of discipline which encourage self control, self-direction, self-esteem and cooperation among children displaying a respect for themselves, their environment, and others with whom they come in contact.?

Staff #1 spoke to children on multiple occasions with a negative and forceful tone that did not model positive behavior when children were not listening. Leadership acknowledged that staff #1 broke behavior management policy protocol.

Complaint:
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On multiple occasions, Staff #1 roughly spoke to children with an aggressive and forceful tone when children were not listening. Leadership confirmed that staff #1 yelled at the children and did not allow children to speak.
Plan of Correction: On 4/25/2025 staff#1 was written up for not complying with our behavioral and classroom management agreement. Staff #1 was not open to coaching and staff #1 is no longer working here.
