Inspector Notes

An unannounced monitoring inspection was conducted from 12:00 pm - 1:45 pm. There were 60 children in care being supervised by 9 staff. The children were observed eat lunch, preparing for nap, and resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
Based on review of four staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for staff A is dated 03/10/2020.
Plan of Correction: Staff A will obtain a new TB screening and submit the results for the record.
Standard 8VAC20-780-270-A
Based on observation of the facility it was determined that not all areas were maintained in safe condition. Evidence: A large amount of peeling paint was observed on the infant room door as well as the supply closet door in the entryway of the facility.
Plan of Correction: Administration has put in a maintenance request to address the peeling paint.
Standard 8VAC20-780-280-B
Based on observation of the toddler room it was determined that not all hazardous substances were kept locked. Evidence: Dish soap and cleaning solution was observed in an unlocked cabinet under the sink of the Toddler room.
Plan of Correction: Hazardous substances were locked during the inspection.
Standard 8VAC20-780-500-B
Based on observation of the facility it was determined that not all procedures for diapering were followed. Evidence: 1. The foot operated trash can in the two year old room was not being utilized. Diapers were observed in an uncovered trash can. 2. Three backpacks and jackets were observed on the changing table in the two year old room. 3. A tablet, basket, and papers were observed on the changing table of the second two year old room.
Plan of Correction: Administration will retrain staff on the proper diapering procedures.
