Inspector Notes
An investigation was initiated from 10:30 am - 12:30 pm with local agency CPS to investigate a self reported incident regarding supervision of children. Interviews were conducted, documentation, video footage, and records were reviewed. Violations were issued to the facility that were associated with the self reported incident. The violation notice is attached.

Based on interviews and review of video footage it was determined that children under 10 years did not always remain in sight and sound supervision of staff as required. Evidence: On 03/21/2023 the Toddler classroom was preparing to go outside to the playground with five children being supervised by one staff. As the toddler class and teacher exit the exterior door they are seen walking down the fenced in sidewalk area to the playground gate. Staff A is seen opening and walking through the gate while 2 children immediately enter the playground with 3 remaining children on the sidewalk by the gate. Two additional children enter the playground. The gate is seen being closed while Child A (Age: 20 Months) is still in the fenced in sidewalk area. Staff A was not aware that child A was not on the playground with the rest of the class. Child A remains in the fenced-in sidewalk area from 3:43 pm - 3:47 pm. At 3:47 pm the gate is seen being opened and staff A retrieves child A and brings them to the playground.
