Inspector Notes

An unannounced inspection was initiated on 07/25/2024 and completed on 08/29/2024 in response to a complaint received by the licensing office on 07/09/2024 relating to allegations of programs and special services. The inspector reviewed two children?s records on-site. Additional documentation was reviewed electronically on 07/26/2024. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/24/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 06/27/24, Child #1 was given a snack containing an allergen. The child?s record contained documentation dated 05/30/24 requesting that the child is not offered meals/snacks that contain the allergen.
Plan of Correction: Per the Center: "Staff will be retrained on allergy procedures on 10/25/24."
Standard 8VAC20-780-40-M
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.

On 07/25/24, the list in the school age room was dated 03/12/24. The center received notification on 05/30/24 that Child #1 had a dietary restriction and sensitivity, and the list was not updated.
Plan of Correction: Per the Center: "Allergy will be updated monthly as needed when new students with any restrictions enroll and when students transition to next program."

Linens shall be assigned for individual use.

On 07/05/24, Child #1 was given a blanket belonging to Child #2 for rest time.
Plan of Correction: Per the Center: "Linens shall be assigned for individual use. Teachers have been coached to ensure that students are getting their assigned linens and if spare linens are used to designate to same child."
