During the inspection, the inspector reviewed the areas listed to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any
serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 07/03/25 and completed on 10/17/25, in response to a complaint received by the licensing office on 06/24/25 relating to allegations of supervision. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/24/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-350-B-1
(Repeat Violation) The required ratio for children ages birth up to 16 months is one staff per four children.

On 06/24/25, Staff #1 was observed supervising 8 children (youngest 3 months old) at approximately 7:45am. A second staff member arrived at approximately 8:04am. It is estimated Staff #1 was over ratio from approximately 7:36am-8:04am.

On 07/03/25, Staff #1 was observed supervising 7 children (youngest 3 months old) at approximately 8:00am. A second staff member arrived at approximately 8:10am. It is estimated Staff #1 was over ratio from approximately 7:45am-8:10am.
Plan of Correction: Per the Center: "Moving forward management and staff will defer care until ratios can be met to ensure compliance. Management & staff will communicate to families at drop off when a classroom is at max ratio as well as the need to defer care until additional staff are in place to meet ratios."

The required ratio for children ages 3 years up to school age is one staff per ten children.

On 06/24/25, Staff #2 was observed supervising 14 children (youngest 3 years old) at approximately 7:45am. A second staff member arrived at approximately 7:58am.

On 07/03/25, Staff #3 and Staff #4 were observed supervising 28 children (youngest 3 years old) at approximately 8:15am. A third staff member arrived at approximately 8:35am.
Plan of Correction: Per the Center: "Moving forward management and staff will defer care until proper ratios can be met to ensure compliance. Management & staff will communicate to families at drop off when a classroom is at max ratio as well as the need to defer care until additional staff are in place to meet ratios."
