During the inspection, the inspector reviewed the areas listed to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any
serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 07/23/25 and completed on 10/23/25, in response to a complaint received by the licensing office on 07/22/25, relating to allegations of administration, staffing & supervision, and programs. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/24/25. A POC submitted after this date will not appear on the public website.

(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #3, who was employed for more than four months, did not have documentation of requesting a sex offender registry check or a central registry check from one state.
Plan of Correction: Per the Center: "Management will ensure that all necessary background are completed before first day of employment."

COMPLAINT RELATED
When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

On 07/22/25, eight school-age children were observed walking around with Staff #1 while the staff passed out breakfast. The staff was also observed pushing around a buggy with four infants. During interviews, it was reported Staff #1 had to regularly pull children from classrooms in the mornings due to staffing issues. Based on the age of the children, the required ratio for the group was one staff to four children.
Plan of Correction: Per the Center: "Management will continue to hire new team members to help with current staffing constraints.
Staff will defer care when the school can no longer safely accommodate students for ratio purposes."

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On multiple occasions, it was observed that in the Pre-K Classroom, staff wrote down children?s names on a board if the children were misbehaving as a threat to take away water play.

On 07/15/25, Staff #2 roughly grabbed and ?got in the child?s [Child #1] face? when the child was exhibiting a challenging behavior.

The following actions or threats thereof are forbidden; roughly handling a child.

Based on interviews, it was determined that Staff #2 roughly grabbed Child #1 (4-year-old) and ?got in the child?s face.? A corrective action issued to Staff #2 stated that ?On 7/15/25 you failed to provide a safe and positive method of guidance while interacting with a child by using handly [sic] a child roughly.?
Plan of Correction: Per the Center: "Corrective action and coaching was given to staff."
